Monte Middlebrooks has been a Failure Analyst Technician with the firm of Bell Evaluation Laboratory since 1989 and has over 20 years of experience in accident site investigation, documentation and sampling; laboratory testing; QA/QC inspection of paints and coatings; and research regarding applicable codes, regulations, and standards. Bell Evaluation Laboratory, a failure analysis laboratory in its present location since 1980, uses a multi-disciplinary approach to determine the various roles that design, manufacture, material, specification and/or procedures may play in the cause of a failure or an unexpected event.Failure analyses performed at Bell Evaluation Laboratory may include testing and event reconstruction when feasible.

Responsible for assisting in site investigation, documentation and sampling. Review of documents produced in litigation to index and catalog according to relevance in a case. Research of literature and other sources such as the Internet, for applicable codes, regulations and standards. Other duties include field surveys, sampling and laboratory testing according to ASTM methods for paints, coatings and other materials.

QC/Shipping Supervisor
KW Industries, Sugarland, TX, 1984-1989

QA/QC inspection of the manufacturing and coating of lighting standards. Set up initial QA/QC program for powder coatings used in manufacturing process. Responsible for incoming materials meeting company specifications and all outgoing shipments being routed, loaded and shipped correctly.

Responsible for handling all aspects of both property and casualty insurance claims from inception to settlement. QA/QC inspection of coatings used in petrochemical and oil field industries. Monitored and reported on tank coatings, pipeline and tubular coatings, including girth weld coating in the field, to make sure application met company specifications. Also conducted laboratory analysis on raw materials and finished products to insure they were within specifications.
